Got short-term disability filing chops?

Anyone have any experience with filing for short-term disability? Cari says now is the time to start the paperwork, since "we would have access to the doctors in the hospital for the required medical signatures, etc." She believes Diane will qualify because of the long recovery she's facing.

Today D-Mo is having a procedure to remove some fluid, but it sounds relatively minor and noninvasive compared with last week.

  3. Elzy, our experience is that there might be someone in the hospital (usually the social worker or in the billing department) who can help with the forms.

  4. If you have a w2 form from an employer, they can file (so I heard), but definitely, like Jim said, people at the hospital should have experience with this.

  5. The form is called a WDS-1 and can be found online at http://www.state.nj.us/labor/tdi/WDS1.pdf

    Diane's SS number will be required and her salary going back TEN weeks. It's a pretty simple form and only ONE Doctor's signature is required.
