Got short-term disability filing chops?

Anyone have any experience with filing for short-term disability? Cari says now is the time to start the paperwork, since "we would have access to the doctors in the hospital for the required medical signatures, etc." She believes Diane will qualify because of the long recovery she's facing.

If you have any tips for filing or know anything about the process, please post to the comments section and I'll get in touch. Thanks!

Today D-Mo is having a procedure to remove some fluid, but it sounds relatively minor and noninvasive compared with last week.

  1. Sending all my Love to Diane. We MacDowell fellows and friends of Diane are spreading the word that she needs our love and support.

  2. Hi Diane,
It's "andretti" here. I'm reading and listening and gathering info about you. I am thinking about you and hoping for the best. I am also available to do anything you guys need from me. love you Andrea

  3. Elzy, our experience is that there might be someone in the hospital (usually the social worker or in the billing department) who can help with the forms.
Diane, I heard the Composers Big Band on BGO early this afternoon on my way down the highway. Swingin'. You be up jammin' soon man. Love, Woodstock.

  4. If you have a w2 form from an employer, they can file (so I heard), but definitely, like Jim said, people at the hospital should have experience with this.
Dear Diane, a barrel full of good wishes from us and my colleagues and pretty much everywhere!

  5. The form is called a WDS-1 and can be found online at http://www.state.nj.us/labor/tdi/WDS1.pdf

Diane's SS number will be required and her salary going back TEN weeks. It's a pretty simple form and only ONE Doctor's signature is required.

Diane, the three Jazz-cateers are pulling for you. Get well soon. Ed, (for) Dave and Dan

  6. From Jill McManus: Diane: Happy St. Patty's Day! We are glad you are here with us, and sending stong thoughts for your recovery and just plain feeling better. Will play one for you Thursday. Love, Jill
